I understand that Heavenly Companionship will be contacting the Employee Misconduct Registry and Nurses Aid Registry for verification of reported misconduct before I can be consider for employment. I understand that the physical requirement of working with Heavenly Companionship includes being on my feet for long periods of time and being able to lift up to 30lbs. I understand a background check will be run on me prior to being offered employment with Heavenly Companionship I understand that Heavenly Companionship is a drug free employer. I consent to any and all requested/required drug screenings I understand that reliable transportation, a valid drivers license, and current driver's insurance are a mandatory requirement of working with Heavenly Companionship I understand that due to the nature of the business, no guarantee can be made as to the schedule or the amount of hours worked. I understand that possible duties can include the following: transportation, light housekeeping, laundry, shopping, running errands, exercise assistance, dementia care, medicine and fluid reminders and other non-medical care.
